Participant(s) listed above, hereby acknowledges participant's voluntary participation in a high-risk activity conducted at the Beaver County Fair [hereinafter "Fair"].  Participant acknowledges being advised that if there is any doubt as to Participant's physical condition, the advice of a competent physician should be sought and followed prior to entry. In consideration of the privilege granted of participation in the Fair event, ad for other good and valuable consideration, Participant hereby agrees to be legally bound, and hereby binds all heirs, executors, administrators, or assigned person and waives and release all causes of action, claims, or rights which Participant may have against Beaver County, the Beaver County Fair Committee, or the agents, representatives, successors, or assignees of the same for any and all injury, physical or emotional harm, or other effects or consequences suffered by me in said activity.  Participant further acknowledges that Participant has received and reviewed a copy of the applicable rules and regulations pertaining to said activity and agrees to abide by them.  Participant acknowledges that Participant has read this document carefully, and that Participant understands its contents, and executes it of his/her own free will and choice.
